The main office of New Jersey Spine Center is located in Chatham, NJ with easy access to all major highways in northern New Jersey. We have multiple additional offices to serve New York State, central New Jersey and the Jersey shore.  Our receptionists are available to answer any scheduling and insurance questions you may have.

Please have available insurance registration information for our receptionists on the telephone or you can download the attached form, complete it at your convenience, and fax it to (973) 635-6254.  Please return this form at least two days prior to your appointment.  The pre-registration process will save you time and will help us to be prepared to receive you.
