What to Bring

Upon arrival, our receptionist will ask for your insurance identification card, appropriate referrals, and your driver’s license or another second piece of identification. Please bring with you any X-rays, MRI, CT-Scans or other applicable studies for review. Please be sure to bring the films as well as the written reports available at the time of your evaluation.

What to Expect

During your initial visit with your physician, you will be asked to provide a complete medical history. A full history and physical examination specific to your complaints will be performed, and imaging studies reviewed. Your physician will summarize your condition, any evaluation necessary and treatment options as decisions are made. Your physician will speak to you about your diagnosis, any necessary evaluations, reasonable treatment options as well as the advantages and potential difficulties with each approach to care.
